Physicians’ Primary Care of Southwest Florida Data Breach Investigation

WHAT HAPPENED?

Recently, Physicians’ Primary Care announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on or around September 17, 2024, Physicians’ Primary Care was alerted to unauthorized access to its information technology network.1 As a result, Physicians’ Primary Care launched an investigation to determine the nature of the incident.

Through its investigation, Physicians’ Primary Care discovered that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party between September 15 and September 17, 2024. As a result, Physicians’ Primary Care began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Protected health information

 

On November 14, 2024, Physicians’ Primary Care posted a notice of the incident on its website. Additionally, Physicians’ Primary Care plans to mail notices to affected individuals. Based on the website breach notice, Physicians’ Primary Care is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
