South West Family Medicine Associates Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating South West Family Medicine Associates, PA (“S W Family Medicine”) regarding its recent data breach. The S W Family Medicine data breach may have involved sensitive personal identifiable information and protected health information belonging to over 36,900 individuals.

S W Family Medicine is a comprehensive health care organization based in Texas.  Focusing on disease prevention and wellness, S W Family Medicine’s Early Detect Program combines an annual wellness exam with early detection testing for chronic diseases such as diabetes, heart disease, stroke, and cancer.2 S W Family Medicine also offers highly specialized services, such as laser hair removal, allergy testing, bioidentical hormone replacement, acupuncture, bioidentical hormone replacement, acupuncture, behavioral health services & life coaching, physical therapy, and weight loss programs and nutrition counseling.2,3 S W Family Medicine has two locations in Dallas, Texas.

WHAT HAPPENED?

Recently, S W Family Medicine reported to the Attorney General of Maine that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on or around August 7, 2024, S W Family Medicine experienced a network security incident.1 As a result, S W Family Medicine launched an investigation to determine the nature of the incident.

Through its investigation, S W Family Medicine confirmed that sensitive personal information in its systems may have been compromised by an unauthorized third-party. As a result, S W Family Medicine began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On October 24, 2024, S W Family Medicine completed this review and determined that personal information was affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Tax identification number or identity protection personal identification number
  • Driver’s license number
  • Date of birth
  • Address and zip code
  • Passport number
  • Medical information (e.g., medication information, medical history, lab results, diagnosis/condition information, etc.)
  • Access information (e.g. passwords, personal identification numbers, numbers or information used to access financial resources, mother’s maiden name, unique private key used to authenticate or sign an electronic record, etc.)
  • Health insurance policy number or subscriber information

 

On November 7, 2024, S W Family Medicine began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to Maine residents, S W Family Medicine is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
