Founded in 1969, OnePoint Patient Care is one of the nation’s only hospice-focused pharmacies capable of bundling a suite of services involving any combination of local dispensing, clinical advice, delivery, mail order, clinical management and network pharmacy solutions.3 Today, OnePoint Patient Care makes thousands of in-home and facility-based deliveries daily, and dispenses and delivers medications locally, providing clinical guidance on calls and managing custom designed disease-state formularies for the hospice partners it serves.3 Headquartered in Temple, Arizona, OnePoint Patient Care has over 100 locations as well as over 55,000 contracted network pharmacies across the country and employs over 200 individuals.4
WHAT HAPPENED?
Recently, OnePoint Patient Care announced that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on August 8, 2024, OnePoint Patient Care became aware of suspicious activity within its network. As a result, OnePoint Patient Care launched an investigation to determine the nature of the incident.
Through its investigation, on August 15, 2024, OnePoint Patient Care confirmed that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third-party between August 6 and August 8, 2024. As a result, OnePoint Patient Care began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Residence information
- Medical information (e.g., medical record number, diagnosis and prescription information)
On October 14, 2024, OnePoint Patient Care posted notice of the incident on its website. Additionally, OnePoint Patient Care recently began mailing data breach notification letters to impacted individuals. Based on the website breach notice, OnePoint Patient Care is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
