Clay Platte Family Medicine is a family medical clinic based in Missouri. Founded in 1995, Clay Platte Family Medicine provides comprehensive medical care, including family medicine, sports medicine, internal medicine, women’s health, procedures, behavioral health services, and pediatrics, to persons of all ages.4 Headquartered in Kansas City, Missouri, Clay Platte Family Medicine has multiple affiliated clinics and locations across Missouri that include Summit Family and Sports Medicine Clinic, Cobblestone Family Medicine Clinic, and Barry Pointe Family Medicine Clinic.
WHAT HAPPENED?
Recently, the Clinics reported to the Attorney General of Maine that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on June 26, 2024, the Clinics became aware of suspicious activity within its network environment. As a result, the Clinics launched an investigation to determine the nature of the incident.
Through its investigation, the Clinics discovered that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party. As a result, the Clinics began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Health insurance information
On October 16, 2024, the Clinics began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to Maine residents, the Clinics is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
