ParkTree Community Health Center Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Pomona Community Health Center, which does business as ParkTree Community Health Center (“PCHC”) regarding its recent data breach. The PCHC data breach may have involved sensitive protected health information belonging to over 40,000 individuals.

ABOUT POMONA COMMUNITY HEALTH CENTER D/B/A PARKTREE COMMUNITY HEALTH CENTER:

PCHC is a healthcare provider based in California. Founded in 1995, PCHC provides a variety of healthcare services, including women’s health, dental care, behavioral health, diabetes care, infectious disease treatment and prevention, podiatry, optometry, telehealth, and chiropractic services. Today, PCHC offers these services to children, teenagers, adults, and seniors living in the communities surrounding Pomona and Ontario, California.3 Headquartered in Pomona, California, PCHC has three additional locations in California and employs over 50 individuals.

WHAT HAPPENED?

Recently, PCHC reported to the Attorney General of California that it had experienced a data breach in which the sensitive personal information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on February 7, 2024, PCHC experienced a network disruption. As a result, PCHC launched an investigation to determine the nature of the incident.

Through its investigation, PCHC learned that sensitive personal information in its systems may have been compromised by an unauthorized third party. As a result, PCHC began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On July 9, 2024, PCHC completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Date of birth
  • Contact information
  • Gender
  • Health plan identification number
  • Medical information (e.g., medical record number, diagnoses, medications, doctor’s notes)

 

On August 21, 2024, PCHC mailed data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, PCHC is providing affected individuals with a list of the specific type of sensitive information impacted.
