WHAT HAPPENED?
On March 25, 2024, Sheet Metal Workers Local Union #83 discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Sheet Metal Workers Local Union #83 determined that an unauthorized third party accessed this sensitive information through an employee’s email account between March 18 and March 26, 2024. On May 24, 2024, Sheet Metal Workers Local Union #83 began notifying individuals whose information may have been impacted. The type of personal information potentially exposed includes:
- Name
- Social Security number
- Address
- Contact information (e.g., phone number, email address)
- Date of birth
- Driver’s license or state ID number
- Passport number
- Financial information (e.g., financial account number, routing number, financial institution name, credit/debit card information)
- Medical information (e.g., treatment/diagnosis information, prescription information, provider name, medical record number, treatment cost)
- Medicare/Medicaid number
- Health insurance information
