First Choice Dental Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating FCDG Management, LLC, which does business as First Choice Dental, regarding its recent data breach. The First Choice Dental data breach involved sensitive personal identifiable information and protected health information belonging to over 228,000 individuals.

First Choice Dental is a privately-owned dental practice based in Wisconsin. Founded in 1996, First Choice Dental offers a wide array of services, including general and family dentistry, restorative treatment, cosmetic dentistry, sedation dentistry, TMJ neuromuscular treatment, youth and adult orthodontics, periodontic and endodontic care, laser dentistry, oral surgery, and more.3 Today, First Choice Dental has 12 locations across Dane County, Wisconsin. Headquartered in Madison, Wisconsin, First Choice Dental employs over 200 individuals.

WHAT HAPPENED?

Recently, First Choice Dental discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, First Choice Dental determined that an unauthorized third party accessed this sensitive information via a ransomware attack on October 22, 2023. On July 16, 2024, First Choice Dental began notifying individuals whose information may have been impacted. The type of personal information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Driver’s license number
  • Government issued ID number (e.g., passport, state ID number)
  • Financial information (e.g., account number, credit or debit card number)
  • Medical information
