ABOUT CORE DENTAL HEALTH:

Core Dental Health is a dental practice based in Tennessee. Today, Core Dental Health is the dental practice of Dr. Jesse Ellsworth. In dentistry, Dr. Ellsworth has practiced at both general practice and pediatric offices.2 He is a member of the American Dental Association (ADA), Tennessee Dental Association (TDA), and Second District Dental Society (SDDS).2 Headquartered in Oak Ridge, Tennessee, Core Dental Health has one location.

WHAT HAPPENED?

On April 24, 2024, Core Dental Health discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Core Dental Health determined that an unauthorized third party accessed this sensitive information on or about March 14, 2024. On July 1, 2024, Core Dental Health began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Dental treatment history 
  • Medicaid number
