The Ambulatory Surgery Center of Westchester Data Breach Investigation

ASCW is an outpatient surgical center in New York state. All surgical staff are Board Certified or in the process of becoming so, and the ASCW itself is accredited by the Accreditation Association for Ambulatory Health Care, among others.1 Clinical specialties of the ASCW include ophthalmology, orthopedic surgery, podiatry, urology, and ear, nose, and throat surgery.1 Located in Mount Kisco, New York, ASCW is clinically affiliated with CareMount Medical, P.C., Putnam Hospital Center, and various Westchester and Putnam surgeons.1

WHAT HAPPENED?

On November 3, 2023, ASCW discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed or acquired. Through its investigation, ASCW determined that between October 23, 2023, and November 3, 2023, an unauthorized third party accessed certain files through an employee’s email account. On June 26, 2024, ASCW began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Date of birth
  • Medical information
  • Health insurance information
  • Financial information
