Greater Cincinnati Behavioral Health Services Data Breach Investigation

WHAT HAPPENED?

Recently, GCBHS discovered it had experienced a data breach in which sensitive personal identifiable information and protected health information may have been accessed. Through its investigation, GCBHS determined that an unauthorized third party accessed this sensitive information on December 10, 2023. On June 12, 2024, GCBHS began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Demographic information
  • Medical information
  • Health insurance information
