Strauss Borrelli PLLC, a leading data breach law firm, is investigating Family Health Center (“FHC”) regarding its recent data breach. The FHC data breach involved sensitive personal identifiable information and protected health information belonging to over 34,900 individuals.
ABOUT FAMILY HEALTH CENTER:
FHC is a community-based health center based in Michigan. Since Moses L. Walker and his colleagues opened the doors for the first time in 1971, FHC has been serving community members who are in need of quality and compassionate healthcare.3 From humble beginnings in a converted trailer to the current state-of-the-art $10 million facility on the same plot of land at the corner of Paterson and Burdick Streets, FHC has become one of the county’s only Federally Qualified Health Centers (FQHC).3 Today, FHC offers comprehensive health services at three locations, with an additional set of mobile health and dental units.3 The services provided by FHC includes adult medicine and pediatrics, internal medicine, obstetrics and gynecology, lab tests and screenings, dental care, behavioral and mental health, substance use treatment, physical therapy, occupational therapy, on-site pharmacies, and vaccinations. In 2023, FHC provided over 89,000 patient-centered medical visits, 21,500 dental visits providing cleaning, procedures, and education, 31,000 mental health visits, and more than 23,000 PT and OT visits.4 FHC is headquartered in Kalamazoo, Michigan.
WHAT HAPPENED?
Recently, FHC discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, FHC determined that an unauthorized actor gained access to this sensitive information on January 25, 2024. On June 5, 2024, FHC began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Medical information
