Omni Healthcare Data Breach Investigation

WHAT HAPPENED?

Recently, Omni Healthcare discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired.  Through its investigation, Omni Healthcare determined that an unauthorized actor gained access to this sensitive information between January 18 and January 19, 2024. On March 19, 2024, Omni Healthcare began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Contact information
  • Medical information (e.g., diagnosis & treatment information, medical record numbers, treatment costs, provider names)
