WHAT HAPPENED?
Recently, Affiliated Dermatologists discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Affiliated Dermatologists determined that an unauthorized actor may have accessed this sensitive information via a ransomware attack between March 2 and March 5, 2024. On April 25, 2024, Affiliated Dermatologists began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

- Social Security number
- Driver’s license number
- Passport number
- Date of birth
- Address
- Medical treatment information
- Health insurance claims information
