Founded in 1966, TPC serves persons with behavioral health conditions (mental health illness and substance use), intellectual and developmental disabilities, and children with developmental delays from birth to three years of age.2 Today, TPC provides quality services to over 10,000 individuals and their families each year in the top 21 counties of the Texas Panhandle with several service centers located throughout the area in Amarillo, Borger, Clarendon, Dumas, Hereford, Pampa and Perryton.2 At TPC, clients and staff decide together on individual plans of care that can include a wide array of available services such as assessment, case management, medication and a variety of other treatment and support services.2 Headquartered in Amarillo, Texas, TPC employs over 200 individuals.
WHAT HAPPENED?
Recently, TPC discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed or acquired. Through its investigation, TPC determined that an unauthorized actor may have accessed this sensitive information on or about October 17, 2023. On May 9, 2024, TPC began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license or state issued ID number
- Medical information (e.g., medical record numbers, diagnosis/conditions information, lab results, medications information, healthcare claims information, clinical/treatment information)
