WHAT HAPPENED?

Recently, Reliable Networks discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Reliable Networks determined that an unauthorized actor may have accessed information belonging to its clients. As a result, Reliable Networks and its clients began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • State of government ID number
  • Passport number
  • Medical information
  • Health insurance policy numbers
  • Medicare of Medicaid numbers
