Strauss Borrelli PLLCa leading data breach law firm, is investigating Affinity Health Services, LLC (“AHS”) regarding its recent data breach. The AHS data breach involved personal identifiable information and protected health information belonging to an undetermined number of individuals.

ABOUT AFFINITY HEALTH SERVICES, LLC:

AHS is a senior management and consulting company based in Pennsylvania. Founded in 1996, AHS provides a continuum of services that includes both comprehensive management and consulting and advisory services to senior living communities with various organizational structures and governing philosophies.3 The services offered by AHS include financial, accounting, reimbursement and compliance services, accounts receivable management and billing services, clinical services, operational services, regulatory services and marketing and referral development services. Headquartered in Indiana, Pennsylvania, AHS employs over 20 individuals.

WHAT HAPPENED?

Recently, AHS discovered that it had experienced a data breach in which sensitive personal information and protected health information in its systems may have been accessed and acquired. Through its investigation, AHS determined that an unauthorized party may have accessed this sensitive information on April 24, 2023. On November 11, 2023, AHS began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Financial information (e.g., account information, credit or debit card number)
  • Medical record number
  • Patient ID number
  • Medicare/Medicaid number
  • Health insurance information 
  • Health information
