WHAT HAPPENED?
Recently, Aveanna discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Aveanna determined that an unauthorized party may have accessed this sensitive information on September 25, 2023. On March 15, 2024, Aveanna began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Driver’s license or state ID number
- Date of birth
- Health insurance information (e.g., Medicare/Medicaid number)
- Medical information (e.g., diagnosis, treatment information, prescription information, medical record number)
