WHAT HAPPENED?

Recently, Crossroads discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information may have been accessed. Through its investigation, Crossroads determined that an unauthorized actor may have accessed this sensitive information through one of its business email accounts on August 25, 2023. On January 17, 2024, Crossroads began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Medical information
  • Health information
  • Medical insurance information
