WHAT HAPPENED?
On September 28, 2023, ESO discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, ESO determined that an unauthorized actor may have accessed and acquired this sensitive information via a ransomware attack beginning on September 17, 2023. On December 12, 2023, ESO began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Address
- Phone number
- Medical treatment information (e.g., injury type, treatment date, treatment type)
