Regional Family Medicine is a family medicine clinic with two locations in Mountain Home, Arkansas. As a family care practice, Regional Family Medicine provides a range of medical services, including pediatric and geriatric care, radiology services, ultrasounds, mammograms, minor surgeries, and laboratory services. In 2017, Regional Family Medicine attained a Level 3 Patient-Centered Medical Home (PCMH) recognition by the National Center for Quality Assurance (NCQA), the highest level of recognition for patient-centered care.3

WHAT HAPPENED?

On June 26, 2023, Regional Family Medicine discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Regional Family Medicine determined that an unauthorized actor may have accessed and acquired this sensitive information between June 8 and June 26, 2023. On December 12, 2023, Regional Family Medicine began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license or state ID number
  • Date of birth
  • Biometric data
  • Medical information
  • Health insurance information
  • Account numbers
