Headquartered in Saint George, Utah, SBHC has 5 health centers in the southwest counties of Utah and employs over 200 individuals.
WHAT HAPPENED?
On March 13, 2023, SBHC discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, SBHC determined that an unauthorized actor may have accessed this sensitive information on or before March 13, 2023. On November 9, 2023, SBHC began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Medical information
