In 1998, SIS founded the Southland Health Center to meet the urgent health needs of the medically underserved local population. Today, SIS provides a variety of health and social services to low-income residents, the elderly, children, and those with disabilities. Headquartered in Garden Grove, California, SIS has one location and employs over 50 individuals.

WHAT HAPPENED?

On October 18, 2023, SIS discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, SIS determined that an unauthorized actor gained access to this sensitive information between October 16, 2023, and October 18, 2023. On November 10, 2023, SIS began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Financial account information
  • Vaccination Status
