WHAT HAPPENED?

On or around July 26, 2023, United Medical Centers discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, United Medical Centers determined that an unauthorized actor may have accessed and acquired this sensitive information on July 26, 2023. On October 27, 2023, United Medical Centers began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Medical information (e.g., diagnosis and treatment information)
  • Health insurance information
