With locations in Anchorage, Eagle River, Wasilla, and Soldotna, Orthopedic Physicians Alaska offers its patients throughout Alaska the latest, most advanced treatment procedures available.5 Founded in 1992, Primary Care Associates consists of a group of family practice providers that offer their patients comprehensive primary care and urgent care services. Headquartered in Anchorage, Alaska, Primary Care Associates treats patients and its three Anchorage office locations in Lake Otis, South Anchorage, and Eagle Creek.
WHAT HAPPENED?
On October 12, 2022, OrthoAlaska discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, OrthoAlaska determined that an unauthorized actor may have accessed this sensitive information on September 6, 2022. On October 11, 2023, OrthoAlaska began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Financial account number (i.e., payment card number, account number, and routing number)
- Medical information
- Health insurance information
