WHAT HAPPENED?
On December 15, 2022, Duininck discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquied. Through its investigation, Duininck determined that an unauthorized actor may have accessed this sensitive information between December 9 and December 15, 2022. On September 19, 2023, Duininck began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Driver’s license number
- Address
- Financial information
- Medical information
- Health insurance information
