WHAT HAPPENED?

On April 17, 2023, The Chattanooga Heart Institute discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, The Chattanooga Heart Institute determined that an unauthorized actor may have accessed and obtained this sensitive information between March 8 and March 16, 2023. On July 28, 2023, The Chattanooga Heart Institute began contacting individuals whose information may have been impacted. The type of information exposed includes: 

  • Name 
  • Social Security number 
  • Date of birth 
  • Driver’s license number 
  • Account information 
  • Health insurance information 
  • Medical information (e.g., diagnosis/condition information, lab results, medications) 
  • Contact information (e.g., mailing address, email address, phone number)
