Headquartered in Dallas Texas, Unified Pain Management has locations throughout the Dallas and Fort Worth area in Dallas, Carrollton, Frisco, and Midland, Texas. Unified Pain Management is comprised of 5 physicians who specialize in diagnosing and treating pain.

WHAT HAPPENED?

On March 21, 2023, Unified Pain Management discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Unified Pain Management determined that an unauthorized actor may have accessed this sensitive information through corporate email accounts on March 21, 2023. On June 14, 2023, Unified Pain Management began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Address
  • Health insurance information
  • Payment information
  • Treatment and diagnosis
