Life Management Center Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Life Management Center of Northwest Florida, Inc. regarding its recent data breach. The Life Management Center data breach involved sensitive personal identifiable information and protected health information belonging to an undetermined number of individuals.

ABOUT LIFE MANAGEMENT CENTER OF NORTHWEST FLORIDA, INC.:

Life Management Center provides behavioral and mental health care services for counties in the Florida Panhandle. Since 1954, Life Management Center has offered assistance with problems including family life adjustment, stress reactions, substance use, and mental health. Included in Life Management Center’s services are mental and behavioral health programs; individual, group, and family therapy; crisis intervention; therapeutic foster care; and a recovery center. To support this range of services, Life Management Center’s staff includes psychiatrists, psychologists, social workers, psychotherapists, counselors, and nurses. Each year, Life Management Center serves over 12,000 individuals and families.

WHAT HAPPENED?

On March 31, 2023, Life Management Center discovered that it experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Life Management Center determined that an unauthorized actor may have accessed this sensitive information on or around March 30, 2023. On July 25, 2023, Life Management Center began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Medical treatment information
  • Diagnosis information
  • Health insurance information
