WHAT HAPPENED?

On May 11, 2023, Advanced Medical Management discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Advanced Medical Management determined that an unauthorized actor may have accessed this sensitive information through an employee email account between May 10 and May 13, 2023. On June 29, 2023, Advanced Medical Management began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Phone number
  • Driver’s license information
  • Health insurance information
  • Medical information (e.g., treatment or diagnosis information, provider name, dates of service, etc.)
