WHAT HAPPENED?

Recently, Grant Regional Health Center discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Grant Regional Health Center determined that an unauthorized actor may have accessed this sensitive information through an employee email account between March 20 and March 24, 2023. On May 23, 2023, Grant Regional Health Center began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Medical information
  • Health insurance information
  • Taxpayer ID number
  • Financial account information
