WHAT HAPPENED?
On March 3, 2022, Pioneer Valley Ophthalmic Consultants discovered that one of its vendors, Alta Medical Management, had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Pioneer Valley Ophthalmic Consultants determined that an unauthorized actor may have accessed this sensitive information between November 13 and November 15, 2021. The type of information exposed includes:

- Social Security number
- Address
- Payment card information
- Medical records
Additionally, on May 11, 2022, Pioneer Valley Ophthalmic Consultants discovered that additional sensitive personal identifiable information may have been accessible through its vendor, Alta Medical Management, until October 26, 2021. The Type of information exposed includes:
- Name
- Email address
- Transaction date and time
- Transaction identification number
- Statement numbers
- Last four digits of payment card or account number
- Information input of comment field
On May 22, 2023, Pioneer Valley Ophthalmic Consultants began contacting individuals whose information may have been impacted in either breach.
