Headquartered in Dayton, Ohio, Graceworks Lutheran Services has over 50 locations throughout southwest Ohio, southeast Indiana, and Northern Kentucky.

WHAT HAPPENED?

On or around February 18, 2023, Graceworks Lutheran Services discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Graceworks Lutheran Services determined that an unauthorized actor may have accessed this sensitive information. On April 17, 2023, Graceworks Lutheran Services began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Medical diagnosis and treatment information
  • Health insurance information
  • Prescription information
