Employing over 690 individuals, Circle of Care has 135 specialized doctors throughout 17 departments. Annually, Circles of Care generates around $40 million in total revenue and holds over $50 million in total assets.

WHAT HAPPENED?

On September 21, 2022, Circles of Care discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Circles of Care determined that an unauthorized actor may have accessed and obtained this sensitive information on or around September 6, 2022. On January 5, 2023, Circles of Care began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Bank routing and account numbers
  • Phone number
  • Medical information (e.g., medical account number, medical diagnosis, procedure codes, service dates)
