WHAT HAPPENED?

On November 7, 2022, West Oak Eyecare discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, West Oak Eyecare determined that an unauthorized actor may have accessed this sensitive information. On January 6, 2023, West Oak Eyecare began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Optical scan images
  • Billing information
  • Address
  • Medical information (e.g., exam results)
  • Health insurance information
