Founded in 1995 by a group of five orthopaedic surgeons, Midwest Orthopaedic Consultants continues to grow and evolve with the aim of providing high quality care to its patients.

WHAT HAPPENED?

On September 29, 2022, Midwest Orthopaedic Consultants discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and obtained. Through its investigation, Midwest Orthopaedic Consultants determined that an unauthorized actor may have accessed and obtained this sensitive information between September 27 and 29, 2022. Recently, in December 2022, Midwest Orthopaedic Consultants began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Medical treatment or diagnosis information
  • Health insurance information
