WHAT HAPPENED?
Recently, Sequoia One discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Sequoia One determined that an unauthorized actor may have accessed this sensitive information between September 22 and October 6, 2022. On December 7, 2022, Sequoia One began notifying individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Contact information (e.g., address, work email address)
- Gender
- Employment status
- Marital status
- Financial information (e.g., wage data related to benefits)
- Medical information (e.g., Covid-19 status, vaccine status)
