WHAT HAPPENED?

On June 16, 2022, Disability Help Group discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Disability Help Group determined that an unauthorized actor may have accessed and obtained this sensitive information on June 13, 2022. On November 18, 2022, Disability Help Group began notifying individuals whose information may have been impacted. The type of information impacted includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Financial information (e.g., account number)
  • Health insurance information
  • Medical information
