WHAT HAPPENED?

On August 5, 2022, Zomo Health discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. During its investigation, Zomo Health determined that this sensitive information was made publicly accessible via the internet between January 15 and August 5, 2022. On September 29, 2022, Zomo Health began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Contact information (e.g., work address, phone number, email address)
  • Health plan incentive information
