WHAT HAPPENED?

On July 16, 2022, First Street Family Health discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, First Street Family Health determined that an unauthorized actor may have gained access to this sensitive information and deleted electronic files containing protected health information. First Street Family Health disclosed that the unauthorized occurred between July 5 and 16, 2022. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Dates of service
  • Medical information (e.g., diagnoses, conditions, lab results, and medications)
  • Health insurance information
  • Billing information
