WHAT HAPPENED?

On September 30, 2021, Northeast Rehabilitation Hospital Network experienced a data breach in which the sensitive personal identifiable information in its systems may have been accessed. Through its investigation, Northeast Rehabilitation Hospital Network determined that an unauthorized actor may have gained access to this sensitive information between September 30 and October 5, 2021. On August 24, 2022, Northeast Rehabilitation Hospital Network began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Address
  • Financial information (e.g., account number, credit or debit card information)
