Initially founded in 1849 for orphaned children, Centerstone has grown to offer services in mental health counseling and care, addiction recovery, child and family services, residential care, crisis services, and more. Today, Centerstone has assisted over 126,800 patients and operates facilities in over 170 locations throughout Florida, Tennessee, Illinois, and Indiana. Headquartered in Nashville, Tennessee, Centerstone employs more than 3,500 individuals.

WHAT HAPPENED?

On February 14, 2022, Centerstone discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Centerstone determined that an unauthorized actor may have gained access to this sensitive information through employee email accounts between November 4, 2021, and February 14, 2022. On August 5, 2022, Centerstone began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Health insurance information
  • Client identification number
  • Medical information (e.g., diagnosis/ treatment information)
