WHAT HAPPENED?
In February 2022, AllOne Health discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information of individuals in its system may have been access. The data breach was discovered when AllOne Health learned that certain wire transfers were routed to a fraudulently created bank account. Through its investigation, AllOne Health determined that an unauthorized actor gained access to an employees’ email account containing sensitive personal identifiable information and protected health information. On July 15, 2022, AllOne Health began notifying individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Driver’s license number
- Health information
