Southwest Health’s facilities include a hospital and two Southwest Behavior Services campuses in Platteville and Cuba City, Wisconsin. Southwest Health offers a range of medical services, including acute care, hospital services, skilled nursing care, rehabilitation, memory care, behavioral and mental health services, and physician clinics. Originally founded in 1900 as a Platteville’s first area hospital, Southwest Health was formed by the consolidation of Platteville Municipal Hospital and Cuba City Medical Center. Today, Southwest Health is headquartered in Platteville, Wisconsin and has over 500 employees.
WHAT HAPPENED?
On January 11, 2022, Southwest Health discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its system may have been accessed or acquired. On July 5, 2022, Southwest Health began notifying patients whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Financial account numbers
- Medical information
- Health insurance information
